We would love to discuss our programs and services further. Please complete the initial intake form and our Intake Coordinator will contact you.
Get Enrolled
Once we establish the best placement for your child, they will either be enrolled in the appropriate services or added to our wait-list. Once enrollment is established, we will send you an enrollment packet to complete, including a basic information sheet, simple questionnaire and a reinforcement inventory. The enrollment packet should be completed and returned along with copies of any previous assessments (diagnostic/psychological evaluation, behavioral assessment, speech and language evaluation, occupational evaluation, IEP documents, etc.) for review prior to your child’s first day.
Secure Funding
In preparation for your child’s first day, you will need to secure funding. Insurance for ABA, Speech Therapy, and Occupational Therapy may apply.
INSURANCE
Mitchell’s Place accepts private pay for services, additionally, health insurance can be filed for most of our services. Our Early Learning Preschool is not covered by insurance. Mitchell’s Place has successfully contracted and is in-network with the following health insurance companies to provide certain services:
- American Behavioral
- BlueCross BlueShield
- Beacon
- Behavioral Health Systems
- New Directions Behavioral Health
- Tricare
- United Healthcare
- VIVA
Insurance coverage will be determined by provider, plan, and type of service. Insurance coverage availability is on a case-by-case basis. Please contact our Intake Coordinator to determine eligibility, info@mitchells-place.com.
